Liposuction removes fat. A tummy tuck removes loose skin and repairs the abdominal wall. That is the whole difference in one line, and it is why the right choice depends less on how much you want to change and more on what is actually causing the shape of your abdomen. If the problem is a layer of fat under skin that still snaps back, liposuction alone is usually enough. If there is skin that hangs or folds, or a bulge from muscles that separated during pregnancy, liposuction cannot fix it, and a tummy tuck (abdominoplasty) is the operation designed for it. Many people have some of both, which is why the two are so often done together.

They are also two of the most common cosmetic operations in the United States. In its 2024 statistics, the American Society of Plastic Surgeons (ASPS) counted 349,728 liposuction procedures, the most performed cosmetic surgery that year, and 171,064 tummy tucks.

Three tissues decide it: fat, skin and muscle

Most of the confusion comes from treating "my stomach" as one problem. A surgeon examining you is really looking at three layers, and each one has a different fix.

What you notice Which layer it usually is What treats it
A soft pocket of fat that diet and exercise haven't moved, skin still firm Fat Liposuction
Skin that folds or hangs over the lower abdomen, even at a stable weight Skin Tummy tuck
A rounded bulge, especially when you stand or strain, often after pregnancy Separated abdominal muscles (rectus diastasis) Tummy tuck with muscle repair
Stretch marks below the belly button Skin Tummy tuck can remove the skin that carries them; liposuction does not
Fat on the flanks or upper abdomen plus loose lower skin Fat and skin Both, often in one operation

Mayo Clinic explains the logic behind the table. After liposuction, the skin molds itself to the new contour: if your skin has good tone and elasticity it usually looks smooth, but if it is thin and not elastic, the treated area may look loose. Liposuction does not remove stretch marks or improve cellulite. A tummy tuck, by contrast, removes extra skin and fat from the abdomen and tightens the connective tissue over the muscles (the fascia) with stitches, which brings separated muscles back toward the midline.

The Colombian Society of Plastic Surgery (SCCP) puts the liposuction side of it the same way: ideal candidates have localized fat with good skin tone, and when the skin is very loose, an excision procedure is needed instead.

What each operation cannot do

Knowing the limits is often more useful than knowing the benefits.

Liposuction will not:

A tummy tuck will not:

Neither one stops future weight gain. Mayo Clinic notes that if you gain weight after liposuction, fat can still build up, including in areas that were never treated.

When the honest answer is "both"

Mayo Clinic says liposuction is often done as part of a tummy tuck to contour the whole abdominal area. Surgeons call the combination a lipoabdominoplasty: liposuction shapes the waist and flanks, and the excision removes the loose skin and repairs the muscle layer in the same operation.

This is common after pregnancy, after a large weight loss, or when years of weight changes have affected both the fat and the skin. If the breasts also changed after pregnancy, the plan may become a mommy makeover, which brings its own recovery and a longer stay; our guide to mommy makeover recovery time walks through it.

Combining is not automatically better. More areas treated in one session means a longer operation and a bigger recovery, so the plan should treat what your examination shows, not everything that could be treated.

How recovery compares, week by week

Recovery is where the two operations feel most different, even though both require a compression garment and a gradual return to activity.

Liposuction Tummy tuck
Operation Small incisions; fat removed through a thin cannula Low horizontal incision, muscle repair, skin removal; SCCP lists 2 to 4 hours, usually under general anesthesia
Drains Not always Usually, to prevent fluid build-up (SCCP, ASPS)
First days Walking from day one is encouraged (SCCP) Walking early, bent slightly at the waist; the SCCP suggests resting with the legs flexed and supported on the first day
Back to daily activities Gradually from the first or second week (SCCP); ASPS lists a return to work around weeks 2 to 3, depending on the job Gradually from the first or second week (SCCP); Mayo Clinic says many people return to work in about 2 to 4 weeks
Bruising and swelling ASPS: bruising usually gone by weeks 4 to 5; Mayo Clinic: the area looks slimmer within several months Swelling settles over months; the SCCP says the final result is visible after about 6 months
What to watch Fluid pockets (seroma), uneven contour; the SCCP notes firm areas (fibrosis) may need specialized massage Seroma, wound-healing problems, scar quality; smoking raises the risk of poor healing

Your own timeline depends on how much is treated and on your health, and it is set by the surgeon who operates.

Does the choice change your trip to Colombia?

Less than most people expect, at least on the calendar. Avianca, Colombia's flag carrier, lists the same minimum of 14 days before flying after liposuction, a mini tummy tuck, a tummy tuck or a lipoabdominoplasty, and 15 days when two or more body procedures are combined. It also asks for a medical certificate issued no more than 10 days before the flight. Those are boarding rules for that airline, and they come on top of your surgeon's clearance, not instead of it.

Other guidance is shorter. ASPS suggests waiting five to seven days before flying after body procedures such as liposuction, and the CDC's travel guidance advises medical tourists not to fly for 10 days after abdominal surgery. If you fly Avianca home from Cartagena, its 14-day line is the one that shapes the booking. The full airline table is in our guide to flying after plastic surgery.

Where the two really differ is in how those two weeks feel:

How to know which one you need before you travel

You cannot settle this from photos alone, but you can get close enough to plan a realistic trip. Before a virtual first consultation, have ready:

  1. Clear photos of your abdomen from the front, both sides and at an angle, standing relaxed.
  2. Your pregnancies and C-sections, and whether you plan more children.
  3. Your weight history over the past year, and whether it has been stable.
  4. Previous abdominal surgery or liposuction, smoking, medications and conditions such as diabetes or clotting problems.

Then ask the surgeon:

Red flags: a recommendation made before anyone has seen photos and your history, a "lipo will tighten your skin" promise, a flight home booked a few days after surgery, or no in-person check before you leave.

Where Dr. Fulvio Correa fits

Dr. Fulvio Correa is a plastic surgeon in Cartagena and a Miembro de Número of the Colombian Society of Plastic Surgery (SCCP). He performs both tummy tuck surgery in Cartagena and liposuction, and decides between them, or on the combination, after examining the abdomen. People who live abroad can start with a virtual consultation through his international patients program; the plan is confirmed in person in Cartagena before surgery, and follow-up continues remotely after you fly home. Consultations take place at his office in Manzanillo del Mar, CC Ramblas local 23.

Frequently asked questions

Is liposuction or a tummy tuck better for a flat stomach? It depends on what is keeping the stomach from looking flat. If it is fat under firm skin, liposuction can do it. If there is loose skin or a bulge from separated muscles, only a tummy tuck addresses those, often with liposuction added to shape the waist.

Can liposuction tighten loose skin? No. Mayo Clinic notes that skin with poor elasticity may look loose after liposuction, and the SCCP advises an excision procedure when the skin is very loose.

Does a tummy tuck remove fat too? It removes the skin and fat in the area it excises, mostly between the belly button and the pubic area. Fat on the flanks or upper abdomen is usually treated with liposuction in the same operation.

Which has the easier recovery? Liposuction, in most cases. Both involve a compression garment and a gradual return to activity, but a tummy tuck adds muscle repair, a longer incision and usually drains. Mayo Clinic says many people return to work about 2 to 4 weeks after a tummy tuck.

How long should I stay in Colombia for either one? If you fly Avianca, plan around its 14-day minimum for liposuction or a tummy tuck, or 15 days if two or more body procedures are combined, plus your arrival days for the in-person evaluation. Your surgeon's clearance comes first.

Can I have a tummy tuck if I want more children? It is usually better to wait. The SCCP says a later pregnancy can undo the result, although the surgery does not affect the baby's development.
